Maybe, but I thought scale, maybe something has killed the scale but we are seeing the effects.
Whatever the cause defoliation, tip pruning and good hygiene should help.

I had also wondered about scale in that there's a scale that affects Euonymus that looks similar, but I also noticed the necrotic tissue in the shots and thought it looked fungal or bacterial.

I'm having the same problem here. No apparent bugs, but the leaves look awful, like bugs have been there. Any other ideas about what might be going on here?

You might try checking the tree in the middle of the night. I once ahd a scale problem on a buttonwood and the only time I could ever catch them was at night. May be worth a try.
