I have just started my journey with azaleas this year so take this with a grain of salt. I think you should cut it down to a few inches above of the viewable trunk and start from there growing back branch structure.

By the look of the leaves, you either have a mineral deficiency of some sort, or an aphid or lace bug infestation.

I was thinking the same as Jim, looks like a lace wing infestation, it needs to be treated. It has been unusually warm (last few days not considered) and I have one of my azaleas infested with the little beasts. Also, like JKD said, it needs to be cut way back to get some branching, if you let it keep going like it is, it will get real "leggy". It might not be a bad idea to go ahead and get the moss away from the trunk, azaleas have really thin bark an are easily damaged by moss growing on them.
